Incoming JCCARE Rescue form JCCARE reps should use this form to submit information about the JCCARE Chin they are fostering. This data will be sent to the webmaster. The information will be posted to the JCCARE website as soon as possible. You will be notified via email when the listing is available for review. Contact update form JCCARE Reps should use this form to submit or update their personal contact information for JCCARE records and your name, email and phone number for the website representative page.		Initial Contact Representatives by State These representatives are responsible for making initial contact on behalf of JCCARE. Communication with shelters, owners, applicants and other rescue groups should be coordinated with them.
Veterinarian Payment Form JCCARE Reps should use this form to request credit card authorization for payment of veterinarian fees. Please send this form at least 3 days prior to the vet visit. This form will be sent to the JCCARE treasurer.		E.O.T. (End of Trail) GUIDELINES Due to the extraordinary veterinary bills JCCARE has been faced with, we find it necessary to revise and restate our policy for E.O.T. Chin. JCCARE will still pay for E.O.T. care, subject to the guidelines contained in this document.
