I, the undersigned, release this Japanese Chin to JCCARE, with the understanding
that I receive no compensation for the exchange, and that the members of
JCCARE,INC, will make every effort to find the very best home for the Chin
from their list of approved homes, as well as providing medical care as
needed.
JCCARE, INC, agrees to assume responsibility for the Chin and to do all that is needed to insure the future well-being of the dog. JCCARE will not sell the dog, nor shall compensation other than donation and adoption fees be received by JCCARE.
JCCARE INC do not act as brokers, but function only as a rescue/placement service for Japanese Chin in need of care and placement.
I, the undersigned, agree that responsibility of the described Chin passes ENTIRELY to JCCARE upon signing this agreement, and further certify that I am the legal owner of the Chin and am therefore entitled to release it to JCCARE.
